Are States Ready to Implement HR 1 and Medicaid Work Reporting Requirements?

This report poses the question of whether states are prepared to meet the new Medicaid work reporting and renewal mandates introduced by HR 1, given ongoing strain from the post-pandemic “unwinding.”

This report analyzes eight Medicaid performance metrics—such as call wait times, application delays, renewal automation (ex parte), and disenrollment rates—to assess which states face the greatest risks under HR 1’s requirements.

The authors highlight that many states already show red flags on multiple metrics, suggesting deep administrative challenges ahead and increasing the likelihood of improper coverage loss. They warn that unless states rapidly strengthen systems, adopt automation, and improve support, millions—including children—could lose health coverage through neither fault nor choice.
